Penalty shoot outs need better management by refs, and tighter rules on behaviour, eg goalkeeper about to face a penalty has to walk straight to the goal line (so no scope for adjusting the ball on the penalty spot or sledging the penalty taker); that goalkeeper is not allowed to touch the ball before the penalty is taken; the goalkeeper has to be ready within 15 seconds of reaching the goal line, if not the penalty can be taken anyway; apart from touching the ball in the act of saving a penalty or trying to save a penalty, the goalkeeper is not allowed to touch the ball in a shoot out. And so on. I recognise that these rules would work in favour of the penalty taker but the penalty taker doesn't normally ponce around delaying things etc.
