My first attempt at photoshop - but you will get the gist, as mentioned I think the horizontal white and black is more Fulham than vertical stripes...

I was thinking a tag line along the lines of

"Mo , Thank you - (you'll) always (be) part of our history"

"Fulham Legend" underneath it

Simply done etc but tasteful and arguably timeless....
