What was the scuffle in the dugout?

I have a pillar in my view and just saw a Fulham player go flying into the dugout then all he'll breaking lose. The ref looked as though he'd lost the plot

One of their thugs launched TC into the tech area. Should have got a red for violent conduct as should Costa for slapping Bryan, in full view of the ref. Look at pic 3 on the Offal slideshow.

That's what it looked like from where I was sat but I thought there must be something I missed because he didn't show a red

A huge push on TC after the ball had gone out. Bearing in mind he was pushed into the dugout it was worthy of a red. Coupled have been  nasty injury.
