If Team a has used all its subs, and team b's player is sent off for a reckless tackle to the extent that the injured player can't continue, is it possible for Team a to use an extra sub?

Rules have changed and I think it may depend on the competition. I think in the league it is no.

Never heard of such a rule.
In the old days before subs the injured player often played out the game limping out on the wing as nuisance value and on occasions scored a deciding goal.
Haven't seen anything like that for ages though.

This is quoted from the rules of the fa cup for 2021/22

In the Qualifying Competition, a maximum of seven substitutes may be nominated. Subject to the below a Club may at its discretion use three of those seven substitute players at any time in a match. In the event that extra time is played in any match a Club may at its discretion use one additional substitute player, (a fourth in total) from the nominated seven substitutes, during such period of extra time. A substitution can only be made when play is stopped for any reason and the Referee has given permission. Prior to the substitution being made the Club must identify to the Match Officials the number of the player out and the player in, in accordance with the Official Team Sheet.

I do not think so but I do think that this would be a good idea but suppose that it would be an open invitation for more cheating from the premier league players who have minimal integrity. but IMO it is a good idea.

Think there was talk about introducing extra subs for head injuries, but otherwise no.

This seems like an inequitable rule.   The original poster sets it up nicely.  But let's take it a step further. Team A has used all three subs and it's only the 60th minute of the last game of the regular season, and this last game will determine whether Team A gets promoted and/or Team B gets relegated.  The score is 1-0 at the moment for Team A.  Team A has to win to get promoted.  Team B needs only a tie to avoid relegation.

Team B's hotheaded dirty player kicks and punches two of Team A's players, causing both of them to be unfit to continue.  Team B's player is of course sent off with a red.  But now Team B has a man advantage for the next 30+ minutes, because there is apparently nothing in the Rules to balance the extreme result for Team A caused by Team B's expelled player.  And if Team B with its man advantage manages to score an equalizer against  the otherwise promoted Team A in those remaining 30 minutes, Team A will rightfully feel cheated out of its promotion.  And Team B will avoid relegation.  Fair?

An interesting argument, but - realistically - Team B's hotheaded player would hopefully be shown a red card after the first offence (the kick?), meaning they would not still be on the pitch to punch someone else.  Although I suppose there could be an immediate melee after the kicking offence which sees punches thrown.  (Shades of the game against Everton with Boa Morte - who was innocent of all charges - some time back).
